WAIVER & RELEASE OF LIABILITY

This waiver applies to all goalkeeper training sessions, camps, and related activities provided by HW Goalkeeping.

I, the undersigned parent or legal guardian of the participant listed in this registration form, acknowledge that participation in soccer and goalkeeper training involves inherent risks. These risks may include, but are not limited to, physical exertion, contact with other participants, use of training equipment, weather conditions, and field or facility conditions, which may result in injury, illness, or property damage, including serious injury.

I understand and acknowledge these risks and voluntarily accept full responsibility for any injury or loss that may occur as a result of participation.

I hereby release, waive, and hold harmless HW Goalkeeping (operated by Helmut Wiebe), its coaches, assistants, volunteers, and any associated facilities, organizations, or property owners from any and all claims, demands, actions, or causes of action arising out of or related to the participant’s involvement in training activities, including any claims arising from negligence, except in cases of gross negligence.

I confirm that the participant is physically fit and medically able to take part in goalkeeper training and that any relevant medical conditions or limitations have been disclosed.

In the event of an emergency, I authorize the coach or staff to seek and obtain medical assistance for the participant if I cannot be reached.

I acknowledge that I have read, understood, and agree to the terms of this waiver
